
Care and Support at Home Redesign Project
A new Care and Support at Home Service is being designed for Falkirk.
The service will introduce a new reablement pathway and a 24/7 urgent response service.
The 1-6 week reablement pathway will help everyone who needs support at home, assessing their needs and helping people live as independently as possible before offering any ongoing personal care package. The urgent response service will reduce demand on emergency residential placements and hospital admissions by offering 24/7 support at short notice - supporting people to leave hospital and those who need short-term help at home.

Survey Results
As part of Falkirk Health and Social Care Partnership’s Care and Support at Home Redesign Project, we have gathered feedback about the current service.
The redesign project seeks to introduce a new reablement pathway and a 24/7 urgent response service. The redesign project is expected to be implemented by the end of 2024.
Between January – March, we asked people who receive support at home, their families, and carers, to provide their experience of the current service. The feedback will be used to help design a new model for care.
The survey was based upon the Care Inspectorate’s own ‘How good is your care?’ survey, which gathers feedback based upon the regulator’s quality frameworks for care services. A total of 27 people (5 supported individuals and 22 relatives or carers) took part in our survey.
This report provides a brief summary of the views of people who completed the survey.
