Falkirk HSCP Locality Plans

Falkirk Health & Social Care Partnership (HSCP) has established three locality areas as part of its commitment to deliver services that meet the needs of the communities. The three locality areas are:

West Locality: Denny, Bonnybridge, Larbert and Stenhousemuir

Central Locality: Falkirk town, Camelon, Bainsford/Langlees and Hallglen

East Locality: Grangemouth, Bo'ness, Braes Villages

The HSCP are required to develop Locality Plans to help identify and address the needs of each Locality area and demonstrate how the Outcomes and Priorities in the Falkirk HSCP Strategic Plan 2023-2026 will be delivered on a local basis.

Work is now underway to develop our three Locality Plans. A steering group of key stakeholders has convened to oversee the development of the Locality Plans. The Steering Group have outlined a high-level Action Plan to develop the Plans.

Locality Delivery Groups for each Locality area will be convened to undertake the development of Locality Plans. The Locality Delivery Groups will include stakeholders specific to the Locality area to deliver the plan.

The Central Locality Delivery Group has been convened and will be the first plan to be developed and published in January 2025. East & West Locality Plans will be developed and published in May 2025. The plans will cover the period of the current Strategic Plan until 2026.

Developing Locality Plans will include reviewing data available to the HSCP on each Locality, utilising the knowledge of operational staff and engaging with the public to hear their views to understand the important issues that the Locality Plans should focus on.

The process undertaken to develop Locality Plans will ensure that health and social care professionals, third sector and independent sectors, housing, service users and carers are given the opportunity to influence and inform service planning and the delivery of our Locality Plans.

Engagement on the challenges within Falkirk Central and proposed priorities to address these challenges is underway. The full set of survey questions is attached below as a pdf. It may be helpful to review the survey questions before submitting a response online. It may also be helpful for those of you who wish to provide a group response.

Desk-Based Research and Information Gathering

Our Falkirk HSCP Locality Planning Steering Group convened and reviewed self-evaluation reports developed by Policy Officers. The self-evaluation reports were developed to demonstrate how we are doing in each locality, the evidence we have for this and suggestions on what could be improved. The self-evaluation looked at five particular themes:

  • Access & Information
  • Assessment & Support Planning
  • Care Co-ordination
  • Prevention & supporting independent living
  • Systems & processes

The Steering Group reviewed this information, including evidence from the third sector and communities in the local area to support with their their decision making to develop an action plan for to develop our Locality Plans.

The Steering Group decided that the Locality Plan to develop first would be the Central Locality with a timeline for it to be completed by December 2024 and approved at our first Integration Joint Board meeting in 2025. A Falkirk Central Delivery Group has been convened to support development of the Locality Plan and has membership from operational staff in the Central Locality and community and third sector.

After the Central Locality Plan has been developed, our East and West Locality Plans will be developed in early 2025 following the same development process as Central.
