What is the HSCP and why are we asking?
The Falkirk Health & Social Care Partnership (HSCP) brings together Falkirk Council, NHS Forth Valley and partners to plan and deliver adult health and social care services. The HSCP Strategic Plan sets priorities for the next few years and explains how services and resources will be planned to improve outcomes for people in Falkirk.
The Housing Contribution Statement (HCS) sits alongside the Strategic Plan and explains how housing services support health and wellbeing — for example through adaptations, homelessness prevention, housing support and accessible homes.
This short survey asks how housing affects health and wellbeing in Falkirk, and what housing priorities should be reflected in the Health & Social Care Partnership (HSCP) Strategic Plan. It should take around 5 minutes. Your responses are anonymous.
If you are also completing the HSCP Strategic Plan consultation, you can still complete this survey — it focuses specifically on housing.
